About Out Pass It is a prepaid medical card, valid for one year, especially created by CML to avoid long, routine, and complicated administrative formalities in order to cover your "out” examinations and give you access to a number of related medical services of your choice (family doctors visit, personal accidents, hospital daily indemnity, dental care). Advantages of Out Pass No more pre-approval No more exclusions No more age limit No more waiting period Cost You can get any of our various Out Pass prepaid cards from our office in Zouk Mikaël and have your "out medical expenses" and other services related to this program covered. An Out Pass prepaid card costs $150 only and is valid for 1 year. Benefits The Family Doctor Outpass 85% coverage of the total amount of laboratory exams and X-rays within the WMC network which is spread out over various Lebanese areas. Unlimited number of visits to any of the CML's family doctors network for a participation fee of 8,000 L.L. for each visit upon prior appointments. The Personal Accident Outpass 85% coverage of the total amount of laboratory exams and X-rays within the WMC network which is spread out over various Lebanese areas. Medical expenses due to an accident. Coverage is limited to $500 per case and a maximum yearly coverage of $5000. The Dental Care Outpass 85% coverage of the total amount of laboratory exams and X-rays within the WMC network which is spread out over various Lebanese areas. The basic treatments are 100% deductable. They include emergencies, consultation, teeth cleansing, amalgam and simple extraction. Any other treatments are up to 60% deductable, as the coverage includes also plastic treatments such as othodontics, whitening, implants, etc. HDI Outpass 85% coverage of the total amount of laboratory exams and X-rays within the WMC network which is spread out over various Lebanese areas. HDI program is a $50 daily compensation due to hospitalization resulting from surgery, accident, or necessary health or illness issues. Yearly coverage limit is $1500. |
